Patient Information

New Patients

New patients should arrive 30 minutes prior to their scheduled appointment. For your convenience, all new patient forms can be downloaded prior to your visit. Or, you may complete the forms once you arrive at the clinic. Please make sure to bring your insurance card with you to the clinic.

Existing Patients

Existing patients should arrive at the clinic 15 minutes prior to their scheduled appointment. If any of your patient information has changed since your last visit, please update our office through the Information Change Form. If your insurance has changed, please bring your new insurance card with you to the clinic. 

Cancellations

Routine appointments (e.g. acute illness, blood pressure check, injuries, etc.) must be canceled at least 4 hours before the scheduled appointment time. Failure to cancel will be reflected as a "no-show" and fees will apply. Our phones are staffed beginning at 7:30 am. Any appointments for the morning must be canceled prior to 8:30 am. 

 

Physicals, procedures, and bone densities must be canceled no less than 24 hours prior to the scheduled appointment time. Failure to cancel will be reflected as a "no-show" and fees will apply. 

No Show Fees

First Occurrence: $25 fee will be charged

Second Occurrence: $50 fee will be charged

Third and Subsequent Occurrences: $100 fee will be charged

Patients who fail to pay the fees for missed appointments will not be allowed to schedule future appointments until the fee is paid. 

Payments for Services

Payment is requested at the time service is rendered unless other arrangements have been made. We accept cash, check and all major credit cards. We will be happy to file insurance claims, but co-pays and deductibles that have not been met are due at the time of service. Please be prepared to show your insurance card EACH visit.

Billing

A separate company, EngageMED handles billing, insurance, and collections for the Little Rock Family Practice Clinic. This company allows you to pay online or by other means. If you have any questions about billing and insurance, please call EngageMED at 501-224-1690 during normal business hours Monday through Friday.
